Why red-cell and plasma rules differ

Red blood cells carry antigens (A, B, and RhD), while plasma carries the antibodies against the antigens a person lacks. For red cells, a donor is compatible when they introduce no antigen the recipient has antibodies against. For plasma the logic reverses: the donor's plasma must not contain antibodies against the recipient's red cells. That is why the two comparisons can give opposite answers for the same pair of ABO groups.

Why O negative is called a universal red-cell donor

O negative red cells carry no A, B, or RhD antigens, so under standard rules there is nothing for a recipient's ABO or anti-D antibodies to react against. This is specific to red cells; O negative plasma is not universal, because it contains both anti-A and anti-B antibodies.

Why AB plasma has broad ABO compatibility

Type AB plasma contains neither anti-A nor anti-B antibodies, so in this standard ABO comparison it can be given to type O, A, B, and AB recipients. This is the plasma counterpart to O negative red cells, and it is why “universal” means different things for red cells and plasma.

Why compatibility does not establish donation eligibility

A compatible result means two types can be matched in principle under standard rules. It does not mean a specific unit is safe to transfuse, or that a person is eligible to donate. Real transfusions depend on antibody screening, crossmatching, product-specific rules, and clinical judgment, and donation eligibility depends on separate health and safety screening.

Can O positive donate red cells to A positive?
Yes, under standard rules. O red cells carry no A or B antigens, and an RhD-positive recipient can receive RhD-positive red cells, so O+ to A+ is compatible.
Can O positive receive O negative red cells?
Yes. O negative red cells carry no A, B, or RhD antigens, so an O positive recipient can receive them under standard red-cell rules.
Can A positive donate red cells to O positive?
No, not under standard rules. A red cells carry the A antigen, and a type O recipient has anti-A antibodies that would react against them, so A+ to O+ is not compatible.
Who can receive AB plasma?
Under standard ABO plasma rules, AB plasma contains no anti-A or anti-B antibodies, so it is compatible with type O, A, B, and AB recipients. That is why AB is described as the universal plasma type.
Does Rh factor matter for plasma compatibility?
Not in this standard ABO plasma comparison. Plasma compatibility here is based on ABO antibodies, so RhD positive or negative is not used to decide plasma compatibility in this tool.
Does this replace crossmatching?
